Nursing Care Homes

For people who need 24-hour nursing care in addition to personal care. Registered nurses are on duty at all times. Eligible residents may receive NHS Continuing Healthcare funding.

Reablement Services

Short-term intensive support following a hospital discharge or period of illness, to help you regain independence at home. Usually provided free for up to six weeks.

Day Services & Activities

Day centres and activity groups provide social stimulation, personal care and support for adults with care needs, helping to maintain independence and reduce isolation.

Direct Payments

Instead of receiving council-arranged care, you can receive a direct payment to arrange your own support. This gives you greater choice and control over who provides your care and when.

How to Access Social Care

Starting the Process

Contact Doncaster Council Adult Social Care on 01302 737391 to request a needs assessment
The assessment is free and you cannot be refused one — it is a legal entitlement under the Care Act 2014
The council will assess your care needs and determine your eligibility for funded support
If eligible, a care plan will be developed with you outlining the support you will receive
A financial assessment (means test) will determine how much you contribute toward the cost
You have the right to a personal budget and to challenge any decisions you disagree with

NHS Continuing Healthcare

If your care needs are primarily health-related rather than social, you may be eligible for NHS Continuing Healthcare (CHC) — which is fully funded by the NHS and not means tested. Ask your GP or social worker to refer you for a CHC assessment.

Carer’s Assessments

If you care for someone with social care needs, you have a right to your own carer’s assessment from Doncaster Council — regardless of whether the person you care for is receiving services.
